Or you may download this form, fill it out & drop it off at the Parish Office, drop it in the collection basket, or mail it to: St. Leo the Great, 4940 Broadview Road, Cleveland, OH 44109. To the right (or below if viewing this from our mobile app), is a brief form for updating your parish registration information.
Those who are moving from the parish are asked to stop in the Parish Office.
